Terms and Conditions for the Longevity Clinic's Health Savings Plan

Welcome to the Longevity Clinic's Health Savings Plan. By enrolling in the Health Savings Plan (the "Plan"), you agree to abide by the following terms and conditions. Please read carefully before enrolling.
1. Overview of the Health Savings Plan
The Health Savings Plan allows you to prepay for treatments and services at the Longevity Clinic in manageable instalments. The goal of the Plan is to help you invest in your health and wellness by spreading out the cost of services over time, making healthcare more accessible.
2. Enrollment and Account Setup
To enroll in the Health Savings Plan, you must complete an online form with your personal and payment information. By submitting this form, you agree to participate in the Plan and accept the terms and conditions outlined herein.
3. Step 1 – Choose Your Service
Upon enrollment, you will select the treatment or service for which you wish to save. Services offered under the Plan are subject to availability and may be adjusted or discontinued at the discretion of the Longevity Clinic.
4. Step 2 – Customize Your Payment Plan
You have the flexibility to choose a payment plan that suits your budget. Available options include:

3-month payment plan

6-month payment plan
-
12-month payment plan

Custom monthly payment amount (with total length of plan determined by your selected monthly contribution)
The length of your payment plan will depend on the amount you choose to pay each month. All payment plans must adhere to the following terms:

Payments are due on the agreed-upon schedule, and any missed payments may result in a suspension of your plan.

Payments can be made via credit card, debit card, or other methods as specified by the Longevity Clinic.
5. Step 3 – Enrollment Confirmation
Once you complete the online form and agree to the terms, your enrollment will be processed. You will receive confirmation of your enrollment via email or other communication channels.
6. Step 4 – Redeem & Enjoy
Once the payment period is complete, you will have 12 months to redeem your accumulated savings for the treatment or service you’ve selected. If you have not saved the full amount required for the service, you have the following options:

Continue contributing during the 12-month window to reach your savings goal.

Pay the remaining balance when you are ready to book your appointment.
7. Expiration of Funds
Any funds remaining in your Health Savings Plan after the 12-month usage window will expire. All savings must be utilized within 12 months of completing your selected payment period.
8. Fund Transfers and Adjustments
While your savings are non-refundable, they may be transferred to another service offered by the Longevity Clinic or to another person. Transfers must be requested within the 12-month usage window and will be subject to availability.
9. Cancellations and Refunds
Once you have enrolled and made payments toward your Health Savings Plan, no refunds will be issued. You may cancel your enrollment at any time, but any payments already made will not be refunded. You will still have the right to use any remaining savings within the 12-month window.
10. Account and Payment Responsibilities
It is your responsibility to ensure your account information is accurate and up-to-date. If any changes occur to your payment method or personal information, you must update your account details immediately. Failure to do so may result in interruptions or delays in your Health Savings Plan.
11. Limitation of Liability
The Longevity Clinic is not responsible for any loss, damage, or delay resulting from technical issues, system failures, or events beyond our control that affect the Health Savings Plan. Additionally, the Longevity Clinic is not responsible for any failure to fulfill appointments or treatment services if the funds in your plan are not sufficient to cover the full cost of the service.
12. Changes to Terms and Conditions
The Longevity Clinic reserves the right to modify these terms and conditions at any time. Any changes will be communicated via email or posted on the website. Continued participation in the Health Savings Plan after such changes constitutes your acceptance of the revised terms.
13. Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the laws of Barbados, without regard to its conflict of laws principles.
